Required Forms for Hajj or Umrah

Original passport valid

For at least 6 months from traveling date (with enough blank pages for visa).

For Non- American citizens,

If applicable, clear COLOR copies of valid green card - front & back.

4 Recent passport-size photos

White background with your name signature on the back of the photos.

Visa Application

Please fill-out the Visa Applicationin INK, one application per person.

Original vaccination certificate

For Meningitis vaccine & H1 N1.

If you are not carrying a Muslim name,

A certificate from the Imam of your mosques indicating that you are a Muslim is required or fill in Declaration of Conversion to Islam form

Husband and wife

Holding different last names, a copy of the marriage certificate is required.

Women traveling alone

(Must be over 45 years) without a mehram, require a NOTARIZED permission letter from the mehram, indicating that he has no objection and permits her to travel for Hajj with Alwaha Travel Agency, LLC. Mehram: Father, Husband, Brother, Adolescent son. Please fill in Notarized Muhrem Letter form

We should receive full payment, along with the above documents upon receipt of your Umrah quotation.
